Orders ship with the wrong item, or none at all: how to fix 3PL fulfillment errors
Most fulfillment errors start in data, not on the warehouse floor. Trace each error to its cause, fix what is yours and hold your 3PL to the rest in writing.
3PL Audit $750 · 3PL coordination in Ops Desk Growth at $2,499 a month
Most 3PL fulfillment errors start in data, not on the warehouse floor: a SKU mapped to the wrong item, a bundle that is never split into its parts, an address nobody validated or an order that never reached the warehouse. Fix them by tracing each error to its cause, correcting what is yours, holding your 3PL to the rest in writing and scoring it every month.
What it looks like
"Orders ship with the wrong item, or none at all."
A customer orders the blue one and gets the green one. Another orders a three-pack and gets one unit. A third never gets anything, because the order sat in an integration queue that failed on Saturday. Each complaint is handled one at a time, so nobody sees that two of the three came from the same broken mapping.
Around the complaints, the same signs show up. Nobody checks 3PL invoices against orders. Stock counts differ between the 3PL and the store. Address errors are caught only after the parcel comes back.
What 3PL fulfillment errors cost
Every error is paid for several times. There is the support conversation, the replacement unit, a second outbound shipment and the return label. Often the returned unit cannot be resold. Marketplace orders add account risk: Walmart's standards include a return rate of 6% or below and a negative feedback rate of 2% or below (Walmart, Jul 28, 2026), and Amazon asks sellers to keep the order defect rate under 1% (Sell on Amazon, selling policies).
Price yours: orders a year × error rate × your cost per error. Build the cost per error from your own numbers:
| Cost of one wrong-item order | Value |
|---|---|
| Support time, two contacts | $6 |
| Replacement unit at landed cost | $12 |
| Second outbound shipment | $8 |
| Return label | $7 |
| Share of returned units that cannot be resold (half of a $12 unit) | $6 |
| Cost per error | $39 |
| Yearly cost at 50,000 orders and a 0.8% error rate | 50,000 × 0.8% × $39 = $15,600 |

That figure leaves out the customer who never orders again, which is real but hard to price.
Trace each error to one of six causes
Each cause leaves a signature in your data. Read the signature before you call the 3PL, because three of the six are yours to fix.
| Cause | Its signature in the data | Fix |
|---|---|---|
| SKU mapping | The same wrong item, again and again, for one listing | Correct the master SKU map |
| Bundle not split | Kits and multipacks ship incomplete | Bundle rules that pick each component |
| Receiving or labeling | Many wrong items traced to one inbound lot | Recount and relabel; check receiving against the advance ship notice |
| Picking | Random errors with no pattern by SKU | Scan-based pick verification at the 3PL |
| Address | Returned to sender, undeliverable | Address validation at checkout |
| Order routing | Order sent to the wrong warehouse, or never sent | Routing rules and integration error alerts |
SKU mapping, bundles and addresses are data problems that live in your store, your channels or your integration. Receiving and picking happen inside the warehouse. Routing can be either, and the integration log tells you which.
How to fix 3PL fulfillment errors, step by step
- 01Log 30 days of errors with order numbers.
Wrong item, missing item, never shipped, wrong address.
- 02Classify each by cause
using the table above.
- 03Fix the data causes yourself.
Correct the SKU map, set bundle rules and switch on address validation at checkout.
- 04Raise warehouse causes with your 3PL in writing, with evidence.
Order numbers, photos from customers and the receiving record.
- 05Add order accuracy and inventory accuracy to a monthly scorecard
tied to the service levels in your contract.
- 06Reconcile invoices and stock every month.
Billing errors and count gaps follow the same root causes as wrong items. Our 3PL invoice audit guide has the checklist.
- 07Decide, with evidence, whether to fix or switch.
What to ask your 3PL
Ask these six questions in writing and keep the answers:
- What was our order accuracy rate by month for the last six months, and how do you measure it?
- How are picks verified: scan at pick, scan at pack, weight check or none?
- What is the cycle count schedule for our SKUs?
- How is receiving checked against our advance ship notices?
- What service level credits does our contract give for errors, and how do we claim them?
- Who is our named contact for errors, and what is their response time?
Then give it 90 days. Month 1: log and classify every error. Month 2: fixes in place on both sides. Month 3: review the scorecard and make the fix-or-switch decision.
Fix or switch?
Switch only when errors are traced to the warehouse, raised in writing and not fixed. Switching has its own cost: exit terms in your contract, moving the stock, a new integration and a learning period in which errors usually rise before they fall. If most of your errors are data errors, a new 3PL inherits them on day one.
What to do yourself first
- Pull 30 days of wrong-item and missing-item complaints.
- Match each one to its order and SKU.
- Check the mapping and bundle rules for those SKUs.
- Ask your 3PL for last month's order accuracy figure.
- Compare the 3PL's stock count with your store's for your top 20 SKUs.
How we run it
3PL Audit is the Relay Module that builds the monthly invoice check, stock reconciliation and service level scorecard in your own tools ($750, built to a written spec, fixes free for 60 days if anything fails acceptance). See 3PL Audit. Stock Flow fixes the SKU and bundle map as part of one stock count across your channels ($1,500 to $3,500).
On Ops Desk Growth ($2,499 a month), our team coordinates with your 3PL every week, runs the monthly checks and chases credits with evidence per order. Any credits recovered are yours; we take no share. Wrong-item returns feed Returns Relay, so the reason codes reach the scorecard. The inventory and 3PL page covers the full service.
Founding offer: 20% off launch fees, add-ons and the Foundation, plus 20% off monthly plans for the first 6 months, for our first 25 clients or until Jan 31, 2027, whichever comes first. See pricing.
How to measure it
Errors per 1,000 orders, split by cause. Stock variance between the 3PL and your store, valued at landed cost. The scorecard measures in your contract. Late or missing scans also show up as "where is my order" tickets, so watch those too: see how to reduce where is my order inquiries. We report these numbers; we do not promise a recovery amount or an error rate.
What does it cost to fix 3PL errors?
3PL Audit is $750 to build, fixed in writing, and sets up the monthly invoice check, stock reconciliation and scorecard in your tools. If you want our team to coordinate with your 3PL every week and chase credits, that is included in Ops Desk Growth at $2,499 a month.
What is a good 3PL order accuracy rate?
The right target is the one written into your contract, measured the same way every month. Ask your 3PL how it measures accuracy (orders shipped correctly ÷ orders shipped) and what its own target is, then compare its report with your complaint log. If the two disagree, your log wins.
Is it always the warehouse's fault?
No. Many errors start in data: a SKU mapped to the wrong item, bundles that are not split into components, addresses that were never validated and orders that never reached the warehouse. Classify each error by cause before you raise it, so the conversation with your 3PL starts from evidence.
Should we switch 3PLs?
Only after errors are traced to the warehouse, raised in writing and still not fixed. Switching costs exit terms, a stock transfer, a new integration and a learning period. If your errors are mostly data errors, the new warehouse will make the same mistakes, because it will receive the same data.
Can you work with our current 3PL?
Yes. We work with your 3PL, not instead of it. We set up the scorecard and reconciliation in your accounts, raise issues with evidence and keep a written record of every request and answer. You keep the contract, the relationship and the decision about whether to stay.
How do we get credit for fulfillment errors?
Through the service level and claims terms in your contract. Most contracts require evidence per order and a claim within a set window. Log every error with its order number, cause and photos, and file monthly. Without an error log, most credits go unclaimed.
Fix the data first. Then hold the warehouse to the rest in writing.
